The quistionairy is a FWSO QSL sample, Fleet Weather and Oceanographic Center, Northwood, England now sendinding out to user via the fax transmissions ,where the users should give a response to the given address.
They now sending back a official QSL Card. Please fill in the boxes if you receive  the station, below and return to:
